Users of the triage system fall into two categories: First Responders (Phone Operators) and Triage Nurses.

Innovative Oncology Business Solutions (IOBS) created The COME HOME triage system to provide aggressive symptom management for patients. It is our hypothesis that aggressive system management has a direct impact on reducing ER utilization and IP admissions & costs.

The Triage System is a real time, decision support web-based system that provides a comprehensive set of triage protocols focusing on patient symptoms. The triage system is populated with patient demographic data directly from an interface written from the Practice’s PMS.

Users of the triage system fall into two categories: First Responders (Phone Operators) and Triage Nurses. Workflow, responsibilities and privileges with respect to the triage system differ between the two roles. Both users access the system and work together to resolve patient–related encounters.

There are two dashboards which help the practice staff manage and assist with incoming patient calls. The first dashboard is called “My Patients.” This dashboard is populated with 36 symptom based pathways, such as Pain, Nausea, and Vomiting. The “My Patients” dashboard tracks the incoming patient related-calls and has the ability to flag customized patient alerts. The dashboard is used by the First Responders to place the patient-related calls on the dashboard for the Triage Nurses. The Triage Nurses take the calls from the dashboard and begin the triage assessment.

The second dashboard is to follow up on patients and is simply called “Follow Up” and is only used by the Triage Nurses. This dashboard is populated with 26 follow-up pathways. The dashboard tracks all patients who need to be followed up with by a phone call.

The Triage Pathways guide the Triage Nurse by providing step-by-step clinical decision-making when discussing and assessing a particular patient problem. The triage system assists users in:
